How to Claim AOA or AMA CreditsAOA CME Submission and Certificate: Complete CME FormOnce you have completed the CME statement of attendance at the end of the online form, you will be directed to a page where you can fill out and print your CME certificate for your records. You will have instant access to fill in your certificate, save and print it. To fill out the certificate accurately, you will need to have the total number of credit hours (noted for each lecture) that you plan to claim. If you select YES to the certificate question, you will be forwarded to a fillable certificate where you can input your information and save it to your device. If you would like to print your certificate, make sure to save the filled-in certificate to your device first and then print. If you select NO to the certificate question, your attestation will be submitted, and you will NOT be forwarded to the certificate page.
